Anticipating the Transition to a Care Setting

Placing a loved one in a care facility is one of the most challenging and emotional choices a family can face on the caregiving journey. This presentation provides supportive discussion about considering a care home and affirms the caregiver’s experience navigating these steps. We will also focus on need-to-know information for exploring care home options and strategies for a successful transition.
