Care Management is an interdisciplinary service that addresses the medical, emotional, social, and practical aspects of aging. At Elder Care Management, we follow the professional standards of the Aging Life Care Association — assessing needs, guiding decisions, and coordinating care so families can navigate aging with confidence.

A comprehensive in-home evaluation of medical, functional, cognitive, and psychosocial needs — resulting in a personalized written Plan of Care with short and long-term goals.

Ongoing Care Management

Continuous monitoring and coordination as needs change over time — including medical appointment coordination, health advocacy, and regular communication with families and providers.

Crisis Intervention & Management*

24/7 availability for urgent situations and emergencies — providing advocacy during hospitalizations, supporting families through sudden health changes, and managing high-stress events.

*24/7 crisis response only available in Greater Sacramento region.

Mediation & Health Advocacy

Support for medication adherence, physician order organization, and care communication — while helping families align on decisions, reducing conflict and confusion during difficult times.

Our Care Managers are experienced and dedicated professionals who bring deep compassion and expertise to every family they serve.

Louie Handugan, MDIV

Director of Care Management

Louie brings over 15 years of hospice leadership to his new role as Director of Care Management. He has guided interdisciplinary teams- including Medical Social Workers, Chaplains, and Therapists- who support patients and families through the emotional, mental, and spiritual complexities of end-of-life care.

Felicia Juntunen, BSW, MA, CMC

Director of Professional Development

Felicia received her Bachelor’s degree in Social Work and a Master’s in Gerontology from California State University, Sacramento. In 2011/12, during her graduate program she served as a student intern with Elder Care Management then officially joined the Care Management team shortly thereafter.

Gail Arno, BS, CMC

Founding Member, Senior Care Manager

Gail brought her expertise to the team at Elder Care Management in 2010, after starting her career in the aging services field back in 1998 as a family consultant for a local placement agency. As a placement agent, she actively educated and guided a generation of seniors and their families to resources and services available throughout the Sacramento area.

Piper Jackson, BA, CDP

Lead Aging Life Care Manager

Piper earned her undergraduate degree from California State University Sacramento and went on to obtain her Residential Care for the Elderly Certificate (RCFE) through the California Department of Social Services. Piper is an active member of the Aging Life Care Association and a Certified Dementia Practitioner through the NCCDP.
