ABOUT CARE MANAGEMENT

What exactly is a Care Manager, and how is that different from a home care aide?
A Care Manager is a trained professional — typically a social worker, gerontologist, or similarly trained professional — who serves as your family’s personal guide, advocate, and coordinator through the often overwhelming world of aging and long-term care. While a home care aide provides invaluable hands-on help with daily tasks like bathing, dressing, and meals, a Care Manager sees the whole picture. We assess your loved one’s complete situation, build a personalized care plan, coordinate with physicians and specialists, and make sure every part of their care is working together seamlessly.
Aging is a journey — and like any journey, it goes more smoothly when you have an experienced guide by your side. At ECM, we work with older adults and their families at every stage of that journey, whether you’re newly navigating the realities of getting older, facing a significant life transition, or managing more complex health needs like dementia, Parkinson’s, heart disease, or recovery from a stroke or major surgery. We meet families wherever they are in the process — sometimes early, when things are just beginning to feel more complicated, and sometimes in the middle of a crisis. Either way, we’re here to help you understand your options, make confident decisions, and make sure your loved one gets the thoughtful, coordinated care they deserve.
A comprehensive care assessment is the foundation of everything we do at ECM. During this in-depth evaluation, your Care Manager meets with your loved one in person to assess their physical health, cognitive functioning, emotional wellbeing, home safety, medication management, and social support. The result is a detailed, personalized care plan that gives your family a clear roadmap — and a trusted partner to help implement it. Most families find it to be one of the most valuable investments they make in their loved one’s care.

That’s a question we hear often — and honestly, the fact that you’re asking it may already be your answer. If you’re feeling stretched thin trying to manage your loved one’s appointments, medications, and daily needs — or if you live at a distance and lie awake worrying about their safety — you don’t have to navigate this alone.

Some common signs that a Care Manager can help include a recent hospitalization or health scare, a new or progressing diagnosis, family disagreements about next steps, or simply the sense that things are getting harder to manage. Our Services

The initial consultation is simply a conversation — warm, unhurried, and completely confidential. We want to hear about your loved one: their health history, their living situation, what’s working, and what’s keeping your family up at night. We’ll ask questions, but more importantly, we’ll listen. By the end of our time together, you’ll have a clearer picture of what care management looks like and whether it’s the right fit for your family. Many families tell us they leave that first conversation feeling a genuine sense of relief — just from having someone knowledgeable in their corner. That’s what ECM has been about since we opened our doors in 2010, and it’s what every family who calls us can expect from day one.
As involved as you want to be. Some families prefer to be closely engaged at every step, while others — particularly those managing from a distance — rely heavily on their Care Manager to handle day-to-day coordination and check in regularly with updates. We adapt to what works best for your family. What never changes is our commitment to keeping you informed, included, and confident in your loved one’s care.
Absolutely — in fact, that’s one of the most important things we do. ECM Care Managers are skilled collaborators who work closely with your loved one’s physicians, specialists, therapists, and home care team. We don’t replace anyone — we connect everyone and make sure the whole care team is working from the same page. Families often tell us that having ECM involved is when their loved one’s care finally felt coordinated.
Care needs rarely stay the same, and neither does our approach. As your loved one’s situation evolves — whether they’re recovering well and need less support, or their needs are increasing — your ECM Care Manager will adjust the care plan accordingly. We’re in this with you for the long haul, and we’ll be honest with you every step of the way about what we’re seeing and what we recommend.

Yes — and this is often where families tell us our involvement made the biggest difference. A health crisis is frightening and fast-moving, and hospitals, as wonderful as they are, are busy places. Important decisions can happen quickly, discharge plans can feel rushed, and it’s easy for families to feel lost in the process.

When you have an ECM Care Manager in your corner, you have someone who can be present at the bedside, ask the right questions, communicate directly with the medical team, and make sure your loved one isn’t sent home — or to a facility — before the right support is truly in place. We’ve been doing this for families since 2010, and we’ve seen firsthand how having a knowledgeable, caring advocate in the room changes outcomes. You shouldn’t have to face a crisis alone.

This is one of the most common and heartbreaking challenges families face, and you’re not alone. Many older adults are understandably reluctant to accept outside help — it can feel like a loss of independence or control. ECM Care Managers are experienced at building trust gently and respectfully with resistant clients. We meet people where they are, move at their pace, and often find that once a loved one gets to know their Care Manager, that resistance softens. We’re also here to support the family while that process unfolds.

Yes — and this is one of the most meaningful roles we play for many families. Long-distance caregiving is incredibly stressful, and the worry of not knowing what’s really happening day to day can be exhausting. Your ECM Care Manager can conduct regular check-ins, attend medical appointments, monitor changes in your loved one’s condition, and keep you updated with honest, clear communication. Many of our most grateful families are the ones who couldn’t be there in person — and trusted ECM to be.

ECM has two offices in Northern California. Through our Sacramento location, ECM proudly serves families throughout the Greater Sacramento region, including communities across Sacramento, South Placer, and the western slope of El Dorado. Through our Vacaville office, we also serve Yolo and Solano counties. We are local — and that matters. Our Care Managers know the hospitals, rehabilitation facilities, home care agencies, and community resources in this region inside and out, so we can connect your family with the right support quickly, without the guesswork.

Our services are primarily private pay. While traditional Medicare does not cover care management, some long-term care insurance policies may provide reimbursements for our service.

ECM’s services are billed on an hourly basis. ECM will charge in increments of one tenth of an hour, rounded off for each specific activity to the nearest one-tenth of an hour. The minimum time charged for any specific activity will be two-tenths of an hour. Some activities such as telephone calls (0.2) and letters (0.2) have a minimum professional charge regardless of the actual time spent.

To streamline the billing process and avoid separate charges, ECM factors travel time into the total billable time incurred for a service visit. This travel time includes the time spent traveling to and from your location to perform our services. Under most circumstances, there are no additional charges or fees for mileage incurred.

We make it a priority to respond promptly — typically within one business day — and we can often schedule an initial consultation within the same week.
