Aging Life Care Management is a relatively unknown resource, this presentation will focus on summarizing the roles and responsibilities of a Care Manager. In addition, this talk will provide
more insights into when Care Management should be considered and specific situations that would benefit from professional Care Management intervention. Participants will gain a better
understanding of Care Management and the profound benefits it provides.
